Issuing Certificates

1. When applying directly at the office

Location and other information

Faculty of Medicine Building No. 2, 1st Floor Educational Affairs (Undergraduate / School of Nursing / School of Midwifery) / Graduate Student Affairs (Graduate School)
Reception Hours: 9:00 to 17:00 on weekdays
※ The office may change its hours or close at the end and beginning of the year or during entrance examinations or other times.

Materials required for application

Personal identification: Student ID, driver's license, passport, health insurance certificate, etc.
Note 1) If the application is made by a proxy, or a proxy is to pick it up, then you will also need a letter of attorney(SAMPLE)from the person applying for the certificate and personal identification for the proxy.
Note 2) If you wish the completed certificate to be posted to you, please bring enough stamps for the return postage. (For postage charges, refer to「"2. When applying by mail.")
Note 3) There is no charge to issue this certificate.

Days required to issue certificates

Japanese-language certificates: About three days.
English-language certificates: About one week (not including weekends and holidays in both cases)

However, since there are a lot of applications at the end of the academic year or each term, there may be delays in issuing them. Please ensure that you apply in plenty of time.

2.When applying by mail

Required materials and other information

We only accept applications from outside the office by mail. (You cannot apply by telephone or fax or other methods)
Assemble the following materials (1) to (3) and post them in an envelope marked 「証明書交付願在中 (CONTAINS APPLICATION FOR CERTIFICATE ISSUE) 」in red on the outside.

②Personal identification
A photocopy of something that proves you are who you say you are (driver's license, passport, health certificate, etc.)
※ When your identity has been confirmed, this will be shredded at the office. If you want it returned, please add a memo to that effect.
③Return envelope
(you do not need this if you are applying by mail but intend to collect it from the office)
Fill in your name and address, and attach enough postage stamps to the return envelope.

Other

If you send the application by mail only and intend to collect the certificate from the office, please bring some personal identification with you. In addition, if the certificate is to be picked up by a proxy, then you will also need a letter of attorney (SAMPLE) from the person applying for the certificate and personal identification for the proxy.
