Those students who hope to graduate from the doctoral program (Doctoral Program of Medicine, Doctoral Program) in the academic year 2017 must submit the Doctoral Dissertation Title Form.
Those concerned should submit the following.


○Who this affects All those students of the doctoral program who hope to graduate in the academic year 2017 (March 2018)
○Documents to submit The "Doctoral Dissertation Title Form" [Excel file]

From Fri, September 1 to 5 pm on Fri, September 8, 2017

○Where to submit

Both an electronic data and the paper should be submitted.
〔1 copy〕You must submit one hard copy to Graduate Student Affairs Section, Faculty of Medicine (医学部大学院係) (1st floor, Building No. 2, Faculty of Medicine) *You may post it, or use the university postal system.*

〔electronic data〕 You must submit an electronic data (Excel file) by uploading it to URL below. Please describe your student ID number and name on the title of the file. *Please do not delete any sheets in the file.*

○Other When you submit the Doctoral Dissertation Title Form, please recommend 7 examination committee members. When you recommend them, please refer to the List of Faculty Member Codes(教員コード表). The List of Faculty Member Codes are included in the “Doctoral Dissertation Title Form” file.

 ・Talk with your research supervisor(指導教員) and nominate, in order of preference, 7 members for these positions in total.
 ・You may nominate up to one person from your own division(専攻).
 ・You may NOT nominate your co-researchers(共同研究者), research supervisor(指導教員), faculty from your lab or department(所属教室の教員), or those related to you by marriage.
 ・You may NOT nominate Dean of Graduate School of Medicine , Director of The University Hospital, nor Director of The Institute of Medical Science, nor Director of The Institute of Molecular and Cellular Biosciences if you cannot obtain the acceptance from them.
 ・You may nominate up to one person from outside of Graduate School of Medicine if approved by the head of the department.
 ・For recommending a member who has noted as 「※」on the remarks column of 「教員コード表」,please contact to Daigakuin-kakari in advance.
 ・In order to ensure fairness of selection, you must nominate 7 members.

1. Launch the computer's Web browser.

2. Enter the following URL. https://ut-gakumu.adm.u-tokyo.ac.jp/websys/campus

3. By referring to the enclosed "Notification of Password", enter your User ID and password after the log-in page is displayed. When logging in for the first time, you will be prompted to change the initial password. Be sure to register a new password of your choice and responsibly manage your new password.
