For those who are scheduled to graduate from the Master's Program in the Academic Year 2012 (Submission of Title Form)
平成24年度修士課程修了予定者の方へ(題目届の提出)
Those who wish to graduate from the Master's Program in the academic year 2012 (March 2013) should follow the instructions below to submit their Master's Thesis Title Form.

| ○Submission period | From Thu, November 1 to 5 pm on Fri, November 30, 2012 *You may post it. |
| ○Where to submit | Daigakuin-kakari(Graduate Student Affairs Section), Faculty of Medicine (1st floor, Building No. 2, Faculty of Medicine) |
| ○Other1 | Refer to "About Preparing the Thesis and Required Materials (Master's Program)" |
| ○Other 2 | Please send the following required materials by email to the Graduate Student Affairs Section as well before the above deadline. Where to send : in@m.u-tokyo.ac.jp Title : 「修士論文題目届(Your Name)」 What to send: 1. Student ID Number 2. Name 3. Division and Department name 4. Contact Information (daytime contact numbers: laboratory extension, cellphone, etc.) 5. Thesis title* * The thesis title will be used in the materials just as it was received, so make sure you have not made any mistakes. In addition, if the title is in English, please append a Japanese translation. |

For those who are scheduled to graduate from the Doctoral Program in the Academic Year 2012 (submission of Title)
平成24年度に博士課程修了を予定している方へ
Those students who hope to graduate from the doctoral program (Doctoral Program of Medicine, Doctoral Program) in the academic year 2012 must submit the Doctoral Dissertation Title Form.
Those concerned should submit the following.

[PDF形式] 「博士学位論文題目届」 [Word形式] [PDF形式] |
| ○Submission period | From Mon, September 3 to 5 pm on Fri, September 14, 2012 *You may post it, but it cannot be submitted to the office of the Institute of Medical Sciences, so please use the university postal system "Gakunaibin". |
| ○Where to submit | Daigakuin-kakari(Graduate Student Affairs Section), Faculty of Medicine (1st floor, Building No. 2, Faculty of Medicine) |
| ○Other1 | When you submit the Doctoral Dissertation Title Form, please recommend 7 examination committee members. When you recommend them, please refer to 「教員コード表」, the List of Faculty Member Codes. ・Talk with your research supervisor and nominate, in order of preference, a total of 7 persons for these positions. ・You may nominate up to one person from your own division. ・You may NOT nominate your co-researchers, research supervisor, faculty from your lab or department, or those related to you by marriage. ・You may NOT nominate Dean of Graduate School of Medicine , Director of The University Hospital, nor Dean of The Institute of Medical Science if you cannot obtain the acceptance from them. ・You may nominate up to one person from outside of Graduate School of Medicine if approved by the head of the department. ・In order to ensure fairness of selection, you must nominate 7 members. |
